Register with us as a New Patient

Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take some considerable time to arrive at the Practice.

After submitting your registration form, it should only take a day or two for you to be added onto our system. Please note that we do not contact you to let you know that your registration has been successful.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

PLEASE NOTE: This practice does not prescribe Methadone (or suboxone), but would refer any patients on this medication to the specialist substance misuse services. Any patients affected by this who wish to register at this Practice must ensure they have alternative arrangements in place until they are seen by the specialist services.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Register online

Step 1: Complete online registration form

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services. Patients on potentially abusable medications may need to see their registered GP prior to a decision being made on whether these medications will be continued or not.

Step 2: Upload scans of photo identity and proof of address (optional)

Please deliver to the practice copies of two forms of your identity to allow us to provide online services, such as repeat prescription requests and appointment requests, when available, via Patient Services.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

Download the Form

If you prefer you can download registration form, via NHS inform and upload to the practice on the form below. If doing so, the below health questionnaire should also be completed. If you are unable to do this please contact the surgery for advice on how to register.

Once you have completed the form please upload the form to the practice.

If you are having any difficulty completing the online form, you can email the downloaded registration form to [email protected] or post them to us. If doing this, please also complete the below health questionnaire, which should be sent along with your registration form.
